Please complete all registration materials before your appointment. You will need to bring a copy of your insurance card, a photo ID, and any pertinent medical records. We also request that you bring in all medications, vitamins, herbs, supplements, and other nutritional products that you are taking to your first appointment.

     Please plan to arrive at least 15 minutes before your appointment time to allow the front office staff time to copy your insurance form and have you sign required release forms. Allow a longer period of time if you have not already completed the registration materials.
